I don't know how common it is but a few people have wiped out the third gear syncro in the Mazda3 5 speed. It is not a cheap or easy fix as the tranny has to be disassembled to replace the parts.

I have seen 3-4th go....but its not as common as 2nd. I have seen the occasional shift fork break....but that leads to NO 3rd or 4th
